Originally developed by Jocelyn Fitzgerald, MD and maintained by the AUGS Scientific Committee, this list includes information and links on validated URPS-related questionnaires. We have identified “free” versus “pay for use” for each questionnaire.

AUGS encourages free usage for questionnaires. Making a validated healthcare questionnaire freely available ensures broad access and promotes equity, allowing clinicians, researchers, and patients across diverse settings to benefit from a standardized, evidence-based tool. Free access also encourages widespread adoption, which strengthens data consistency, improves clinical decision-making, and supports better patient outcomes. Removing financial and administrative barriers accelerates integration into routine care and research, ultimately advancing the field as a whole.
